JOB SUMMARY

Job SummaryThe Public Health Nurse Immunization Program Coordinator's core focus is immunization for vaccine-preventable disease and vaccine management. Expertise is utilized for assessment, program planning, education, evaluation, and treatment of individuals and communities, with a focus on ensuring health equity for BIPOC, immigrant, and rural communities, people experiencing houselessness, adults in custody, and other focus population. Diseases of focus may include COVID-19, childhood vaccine-preventable diseases, Hepatitis, Influenza, and other communicable diseases as required. Provision of clinical services may occasionally occur in non-clinical settings, including outdoors in a range of weather. -
Please Note:This position may be filled at the Health Care Professional 3 or Health Care Professional 5 level depending on experience, education, and specialized certifications. -

  • HCP3 Salary: $64,289 - $92,978 -
  • HCP5 Salary: $72,114 - $104,294 -This position requires occasional evening/weekend work.This position may also be called upon to assist with emergency response.The Public Health Division values a healthy work-life balance and supports a collaborative team atmosphere. -This position is set to decrease to 0.60 FTE on June 30, 2025. Additional funding will be sought to permanently keep this position at full-time beyond the 23-25 biennium. - for a complete list of the duties, responsibilities and physical requirements of this position.

    Community Health Centers of Benton and Linn Counties (CHC)
    The Community Health Centers of Benton and Linn Counties (CHC) opened as an FQHC in 2004. It is the medical home for over 9,200 patients across six clinics. It serves a patient population that is diverse in age, ethnicity, and socio-economic status. The Community Health Centers provide physical, behavioral, and oral health care as well as addiction services. -Our services aim to be culturally and linguistically appropriate. The CHC accepts Medicaid, Medicare, and private insurance. A sliding fee scale and assistance accessing coverage is available for people without insurance. -

                    The following minimum qualifications are required for this position: -

                    Health Care Professional 3 level: -
                    • -RN with Bachelor of Science in Nursing (BSN) from an accredited university; AND
                    • -3 year of nursing experience
                      Health Care Professional 5 level:
                      • RN with Masters of Science in Nursing (MSN) from an accredited university; AND
                      • 8 years of nursing experience - -An equivalent combination of education and experience may be accepted. - -

                        -Special Requirements:Currently licensed in Oregon as a Registered NurseSuccessfully pass a criminal background checkValid Oregon Driver's LicenseAble to work on site or off site per assignments/commitments. -The Ideal Candidate:
                        • Experience immunizing children
                        • Experience using state immunization registry.
                        • Experience assessing vaccination records and administering vaccinations.
                        • Experience using electronic medical records (EMR)
                        • Experience leading a diverse team to ensure quality of care and compliance with requirements.
