Summary: The Care Coordination Clinic Assistant supports patient care in accordance with established methods and techniques and conforms to recognized standards of The Corvallis Clinic and the Medical Home Model of healthcare. The Assistant will support Care Coordination, the Clinical Pharmacist, the Clinical Director, and the Diabetes Education program. This position will report to the Manager of Care Coordination. Responsibilities: Will participate and maintain a culture within The Corvallis Clinic that is consistent with the content outlined in the Service and Behavioral Standards Handbook. To this end, employee will be expected to read, have familiarity, and embrace the principles contained within. Answering the phones for Care Coordination and managing the team emails and schedules Schedule patients for Diabetes Education Class Coordinating group classes and group class preparation Facilitate primary care patients in locating community assistance for psychosocial needs and social determinants of health. Follow up with these patients to ensure progress. Maintain the TCC online community resource directory by keeping it up to date and organized Ensure Payor requirements are met for Diabetes Education referrals Work with the Collective system to ensure primary care patients are tracked upon d/c from hospitals and skilled nursing facilities Manage and carryout Emergency Department project for patients who are frequenting the ED Importing documents into Athena from outside systems for ED visits and other Keeping minutes for various meetings Participating as a member of a multidisciplinary team Drafting letters and documents in support of Care Coordination or Diabetes Education patients Assist with internal marketing materials for various programs Stocks/Orders supplies for Diabetes Education and Care Coordination Participate in daily care coordination huddle Assist with Patient Family Advisory Council Continue to develop position into proactive, evidence-based role with prevention, cost saving, Medical Home, and health promotion in mind Other duties and office support as necessaryEducation/Licensure/Experience: Associates degree and 1 year experience working in a fast-paced Clinic or office setting in an administrative or coordinator capacity, or 2-years experience working in a fast-paced Clinic or office setting in an administrative or coordinator capacity required. Knowledge and Skills: You must have the ability to work well with patients, providers, and other staff along with the ability to handle multiple tasks simultaneously and cope with the obstacles of a busy, fast-paced work environment. You must be motivated, a self-starter, detail oriented, a critical thinker, and organized. Competent with computer and Microsoft suite. You must be an excellent communicator verbally and able to draft letters and other written documents. Prefer experience working with medical records systems. You must be enthusiastic about working in an ever-changing healthcare environment and wanting to serve to the best of your ability as a member of a health care team.
