As a member of our interdisciplinary team, you'll work closely with patients, families, and caregivers to address complex medical and social needs.Responsibilities

• Conduct comprehensive assessments and develop personalized care plans
• Manage patients through acute medical situations and chronic condition exacerbations
• Collaborate with the care team to ensure accurate and complete documentation
• Communicate effectively with patients, families, and caregivers to provide education and support

  • Active and unrestricted licensure in the state of practice
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
  • Board certification in a relevant specialty
  • Ability to work independently and collaboratively as part of a teamPreferred Qualifications
    • 1+ years of clinical experience in senior living, geriatric medicine, or home care
