Job Description:
Providence Connections Palliative Care is seeking a
board-certified Nurse Practitioner to join a well-established,
trans-disciplinary team providing palliative care to patients and
their families in primarily outpatient setting. This role will
provide evaluation and management of chronic conditions or serious,
life-limiting illness. The ideal candidate will have previous
palliative, oncology, and/or geriatric care experience. Connections
Palliative Care is part of a growing integrated Home and Community
Services program which collaborates with Hospice, Home Health,
Geriatrics as well as hospital-based practitioners to care for
patients and their families.
- Part-time at 0.7 FTE, benefited position
- Monday - Friday work week, daytime hours
- Collaborate with physicians, nurses, social workers, and
chaplains
- Minimum of one year of post-graduate clinical experience in
palliative care, hospice, geriatrics, and/or oncology care is
preferred
