  • Graduate of an accredited nursing program
  • Minimum of one (1) year of recent experience in acute care or home health care
  • Current LPN licensure in the state of Oregon
  • BLS/CPR certification
  • Valid driver's license and auto insurance
  • Strong verbal and written communication skills
  • Ability to meet physical demands including standing, lifting, and bendingPreferred:
    • Associate's degree in nursing
    • Experience in home health careKey Responsibilities:
      • Provide patient care under the supervision of a clinician, ensuring that care plans are followed accurately
      • Monitor and document vital signs, medical histories, and patient conditions
      • Educate patients and caregivers on health management and home care techniques
      • Coordinate with other healthcare professionals (HHA, OT, PT, MSW, ST, etc.) to ensure comprehensive patient care
      • Maintain accurate clinical documentation according to the care planWork Details:
        • This is a traveling position, requiring a valid driver's license and a reliable vehicle
        • Flexible schedule: 5 days per week with one weekend per month on call
        • Work remotely with opportunities for field-based care
