Registered Nurse (RN) | Home Health Location: Mount Angel, OR Agency: Revolution Healthcare Services Pay: $2,672 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/16/2026 About the Position Job Opportunity – Home Health RN – Oregon Start Date: February 16, 2026 Location: Oregon Compensation: Details provided to qualified applicants Schedule: 8-hour day shifts, 5 days per week. May visit up to 6 patients per day with a 1:1 ratio and 30 productivity points standard. Some weekend shifts may be required (0-8 weekend shifts per contract). No on-call required. Highlights: Serving adults and geriatric patients in a home health setting with a daily census of 415 Epic charting system utilized for documentation Collaborative interdisciplinary team including physical therapy, interpretation services, and social services Efficient patient scheduling minimizes drive time; occasional visits may extend to a 50-mile radius Free parking and IRS standard mileage reimbursement provided Flexible scrub color policy Requirements: Minimum 1 year of skilled home health experience required OASIS experience required Current BLS certification Epic charting system experience preferred Reliable personal transportation required COVID-19 primary vaccination series plus one booster required Skills must include: admission/discharge assessments, start of care, wound care, medication management, pain assessment, IV therapy, phlebotomy, and infection control Minimum of 2 supervisory references covering 1 year of comparable home health experience within the last 3 years Why It's a Great Opportunity: This home health position offers the autonomy and fulfillment of one-on-one patient care in their own homes, while being supported by a well-coordinated team that values efficiency and quality care delivery.
