Job Description:
Legacy Health, a leading non-profit health network in the
Pacific Northwest, is seeking a compassionate and skilled Clinical
Social Worker to join our dedicated healthcare team. This vital
role involves working directly with patients and their families
across various departments at Legacy Health, providing psychosocial
support, counseling, and resource guidance to enhance the
well-being of each individual under our care. The successful
candidate will be adept at handling complex cases and delivering
patient-centered care that harmonizes with our commitment to
excellence and community service. Conduct patient assessments to
determine psychosocial, mental health, and emotional needs. Develop
and implement personalized care plans in collaboration with
healthcare teams. Provide crisis intervention and facilitate group
as well as individual counseling sessions. Assist patients and
families in navigating health systems and accessing community
resources. Maintain documentation and comply with clinical and
legal requirements. Strong interpersonal and communication skills
Experience in crisis intervention Ability to work collaboratively
in a multidisciplinary team Knowledge of healthcare systems and
resource networks Proficiency in patient assessment and care
planning Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Licensed Clinical Social Worker (LCSW) in the state
of practice Certification in Clinical Social Work (Optional but
advantageous)
