Job Description:

Legacy Health, a leading non-profit health system located in Oregon and Washington, seeks a dedicated Case Manager for our Resource Team within the Community and Social Services department. This pivotal role involves managing a portfolio of cases where you will ensure patients receive comprehensive and compassionate services across our network. You will work closely with healthcare professionals to facilitate patient care coordination, discharge planning, and ensure patient advocacy in a multidisciplinary setting. This position requires strong organizational and communication skills, as well as a heartfelt commitment to enhancing patient well-being. Assess patient needs and develop individualized care plans Coordinate with healthcare providers to ensure seamless delivery of services Manage patient records and document all case interactions Facilitate communication between patients and healthcare teams Ensure compliance with healthcare regulations and standards Provide crisis intervention and support as needed Educate patients and families about resources and support available Assist in the design and implementation of health programs within the community Strong interpersonal communication skills Ability to multitask and manage priorities Detailed knowledge of healthcare systems and policies Proficient in Microsoft Office and case management software Crisis management abilities Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Case Manager (CCM) preferred Valid driver’s license as travel may be required
