Job Summary

Benton County is looking for a Bilingual Clinical Health Navigator to provide assistance to our beautiful community! If you love to help people and have a background in Public Health, Education, Community Outreach, Health Services or Health Navigation we'd love to hear from you!

JOB SUMMARY
Health Navigators work as part of an interdisciplinary team to conduct community level outreach, education, and navigation services. They conduct organizing and mobilization to engage communities and organizations in health planning, program development and policy activities. They may provide specialized services depending on their area of assignment. This position will provide services throughout Benton and Linn Counties, specifically at the Community Health Centers of Benton and Linn Counties, as well as community partners in the area. Health Navigators provide clinical and social health navigation services, care coordination, self-management education, outreach, enrollment and engagement for our Benton and Linn County patients and surrounding communities.

Please Note: This is a Bilingual Spanish/ English position and offers a Bilingual Differential of base salary plus an additional 5% upon successful completion of required language testing.

                  • This position guarantees two salary raises within your first year.                       MINIMUM QUALIFICATIONS
                      The following minimum qualifications are REQUIRED for this position:
                      • 1 year of education or technical training beyond High School; AND
                      • 1 year of work experience in Public Health, Education, Community Outreach, Health Services, or Health Navigation. - OR -

                        • Associate's or Bachelor's Degree in Public Health, Community Health or related field; AND
                        • 6 months of work experience in Public Health, Education, Community Outreach, Health Services, or Health Navigation. An equivalent combination of education and experience may be accepted. Please note candidates must have at least six months of actual work experience to be considered as equivalent.
                          Special Requirements:
                          • This position is Bilingual and requires duties to be done in both English and Spanish. Successful completion of language testing is required.
                          • Valid Oregon driver's license. The Ideal Candidate
                            • Has knowledge and experience connecting with community and social services in Benton and Linn County.
                            • Has experience planning and conducting outreach to hard-to-reach populations.
                            • Has experience using health literacy principles.
