As a Discharge Coordinator joining our team, you're embracing a vital mission dedicated to making communities healthier ® . An Inpatient Therapist who excels in this role: Conducts individual and group therapy sessions to educate patients regarding psychological, emotional, or substance abuse problems. Displays active involvement in treatment planning process. Works well with the interdisciplinary team including physicians, utilization review and nursing staff members. Completes all documentation thoroughly, timely, and in accordance with hospital standards.
